Diabetes Self Management

Diabetes Self-Management is a series of classes intended to educate and empower individuals living with diabetes and those supporting them.  Classes are taught by a multidisciplinary team of licensed healthcare professionals from Mammoth Hospital. 

  • What is Diabetes?

    This introductory class will provide an overview of the pathophysiology, categories (prediabetes, gestational, type 1/1.5/2), and methods of diagnosis, monitoring, management, and treatment of diabetes.

    Learn ways to delay or avoid common health complications of diabetes. Meet others living with diabetes, ask questions, and discuss experiences.

  • Healthy Eating

    Making healthy food choices is central to managing diabetes. Blood glucose levels are deeply influenced by the type, amount, and timing of food ingested. Participants will learn major sources of carbohydrate, protein, and fat and how to plan a balanced menu of recommended portions and time intervals.

  • Monitoring & Medication

    Blood glucose levels may be affected by a variety of factors including nutrition, physical activity, stress, and infections. Blood glucose monitoring is a crucial element to understanding your diabetes, tracking general patterns and overall disease progression, and developing an effective management plan. Discuss different tools available for monitoring and learn the benefits and risks of common diabetes medications.

  • Staying Active

    Physical activity lowers blood glucose levels, helps maintain a healthy body weight, and improves cardiovascular health and blood pressure. Learn elements of a safe and effective exercise routine and develop physical activity goals for diabetes management.

  • Healthy Coping

    Living with diabetes can be overwhelming. Learn how to decrease stress and improve your outlook. Learn healthy coping behaviors and skills and connect with resources for continued support.

  • Problem Solving

    Obstacles and setbacks are inevitable. Learn strategies for dealing with diabetes related problems. Identify common barriers and develop problem solving skills and backup plans to support your long-term goals. Resources for continued support are also provided.

Price:

Single drop-in session: $25

6-session series: $120 (equivalent of $20.00/session)

Diabetes Self Management Package (6-session series + Elevate 5-Class Pass): $135 (click here to purchase)

Some costs associated with Diabetes Self Management may be covered by certain insurance plans. We do not bill your insurance directly for classes. You can pay out of pocket and ask your health insurance plan what steps you need to take to get reimbursed. Often a letter of medical necessity from your doctor is required when seeking reimbursement. You may also be able to use your employer-provided flexible spending account (FSA) or health savings account (HSA) to help pay for some expenses with pretax dollars.

Requirements:

Participants younger than 18 years of age are required to have parental consent. This course is not open to individuals younger than 14 years of age at this time.

You should receive a confirmation email with class details upon successful sign up. (Please be sure to opt into scheduling and reminder emails as this is how we will notify you of class cancellations and schedule changes as well).

Missed the course start?

We have open enrollment so you can still sign up for an unfilled course and catch the remaining sessions! You may also join the waitlist and we will contact you when future course dates become available.

Need to miss a workshop?

Participation in all 6 sessions is highly encouraged but not required. Each session can stand alone and participants will come away from each session with beneficial training tips. We also have drop-in pricing for participants who are only able to attend 1-4 sessions.

Cancellations:

Sign up early! Courses with fewer than 3 participants enrolled 24 hours in advance are subject to cancellation.

Notify us of plan changes early! Participant course cancellations with less than 24-hour notification are subject to charge.
